A message from Alberta Health Services

Hello parents and guardians:

We were excited to welcome students back into the building on September 1. It was wonderful to see smiling faces in the hallways once again and to witness excited engagement in learning experiences.

We have received a request from Alberta Health Services to share information about upcoming COVID vaccination clinics with you, as well as a vaccine consent form. A COVID-19 vaccination program for youth aged 12 to 19 will be taking place in schools, organized by Alberta Health Services. We are sharing information on their behalf.

If you are not interested, please do not fill out the consent form.

The vaccine consent form, linked below, must be filled out in order for vaccination to occur. This is for children over the age of 12 who have not yet had two doses of immunization against COVID-19. COVID-19 School Immunization Consent Form

If you would like your child to be vaccinated, please send the completed form back to school no later than September 15.  You may send the form via email to janice.hailes@ngps.ca, send it back with your child, or drop it off in person at the office. Once AHS has gathered the information, they will organize the date of a vaccine clinic in our school, likely before mid-October.

If you do not want your child to participate, or if your child is already fully vaccinated, there is no need to complete the form. COVID-19 vaccination is completely voluntary and no children will be vaccinated unless the consent form is returned.

We have missed the many events that kept our school busy throughout the year. Everyone is looking forward to increased activities in the year ahead!
